I, ________________________________________________, (please print name) have voluntarily elected to participate in the activities, meetings, social gatherings, volunteer opportunities, parties and PHlockings hosted by East Tennessee Parrot Head Club. I understand that I am responsible for my own actions and behavior and shall treat each other members, guests, hosts, and their personal property, with respect. I agree to abide by all local, state and federal laws.
This agreement releases East Tennessee Parrot Head Club from all liability relating to injuries that may occur from all activities, events or gatherings at any and all locations. By signing this agreement, I agree to hold East Tennessee Parrot Head Club and all of its Directors, Officers, members, employees, volunteer workers, insurance carriers, agents and staff entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence or not, from any and all responsibility for my participation in all work and other activities I choose to attend or participate in.
If I choose to drink alcohol I understand that the East Tennessee Parrot Head Club and all of its Directors, Officers, members, employees, volunteer workers, insurance carriers, agents and staff will not be held accountable for any injuries sustained while under the influence.
I also acknowledge the risks involved in attending all events that the East Tennessee Parrot Head Club may participate in. These activities may include, but are not necessary limited to, water sports, boating, hiking, driving, swimming, kayaking, canoeing, kitchen duties including cooking and other activities including recreational, volunteer opportunities, and or physical activities.
I swear that I am participating voluntarily and at my own risk. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in these activities.
By signing below I forfeit all right to bring a suit against East Tennessee Parrot Head Club and all of its Directors, Officers, members, employees, volunteer workers, insurance carriers, agents and staff for any reason. I will also make every effort to obey any safety precautions and to use my best judgement to prevent injuries while participating in these events.
I represent that I am of sound mind and body, and am fully capable of understanding this waiver agreement. I signed this Waiver freely and voluntarily, without any inducement or coercion.
